From years of experience dealing with clients and insurance companies here are some useful tips on how to claim on Czech private health insurance.
The first and foremost rule is call the assistance service before you use the insurance.
The assistance service should be quite helpful and they can give you the address of nearby doctors.
Calling the assistance service will smooth out refunds, payments and reduce problems. With some of the companies it is written in the terms and conditions that you should call the company before you use the insurance.
Note: Yes, it is maybe a pain dealing with the help line first. But, they can confirm your insurance is valid and help guide you through the claims process. The helpline can tell you not to use the Canadian Medical Center and can tell you places where you can use the insurance without paying.
Not calling the assistance service will result in delayed refunds.
Have your health insurance card ready, you will be asked for your name, type of insurance and contract number as a minimum.
Call the relevant number listed below. Most likely you will hear a recorded message in czech language. Do not worry, stay on the line and there will be a sentence such as "For communication in English Press 4".

Someone will answer, probably in Czech. Just ask to speak in Engish (or whatever language) and they will oblige.
With PVZP you should always use PVZP contracted doctors and clinics.
When you use PVZP cntracted clinics should be nothing to pay (although this is not always the case, even after talking to the assistance service).
If you get a bill from a doctor or clinic make sure you have a recipet and then contact the assistance service and ask them about a refund.

With Axa they have a very short list of contracted doctors or clinics. You should not have to pay anything at a contracted clinic.
With Axa you will most likely pay cash for your treatment. Tell the doctor or clinic you will be paying cash, this should not be a problem.
Ask for a receipt and claim the money back by contacting the assistance sevice.
