How to use Maxima health insurance

You should always contact Maxima before you use the insurance (excluding emergencies)

You will need to know your contract number before you call.

Call the assistance service and they will confirm the insurance is valid (especially for first time use).
The assistance can recommend a medical service near where you live or work.
Calling the assiatnce service will smooth out billing and payments.
Maxima especially recommend that you call the assistance service if you plan on using non Maxima medical services.

How to claim with Maxima

If you have visited a medical facility and paid for the treatment in cash, you must send the following documents to the insurance company:
original proof of payment,
a medical report,
bank account number,
a completed Notice of Claim form

Payment with Maxima

Maxima can accept intertantional bank trasfers.
From the Maxima website:
Please transfer the sum to the bank account number 209855325/0300; the transaction identification number is the number of your contract specified in the head of the contract. IBAN CZ59 0300 0000 0002 0985 5325 and SWIFT CEKOCZPP need to be completed in case of payments from countries other than the Czech Republic.
Note: There will be a delay between payment and recieving the insurance documents.
Insurance companies email the insurance documents once the payment has cleared, this can take some days.
